A Case Report（CARE）is a detailed narrative that describes，for medical，scientific，or educational purposes，a medical problem experienced by one or several patients.①

Case reports are common and account for② a growing number of articles in medical journals；however，their quality is uneven.For example，one study evaluated 1，316 case reports from four peer reviewed emergency-medicine journals and found that more than half failed to provide information related to the primary treatment that would have increased transparency and replication③.Written without the benefit of reporting guidelines，case reports often are insufficiently rigorous① to be aggregated for data analysis，to inform research design or to guide clinical practice.

Reporting guidelines exist for a variety of study designs including randomised controlled trials（Consolidated Standards of Reporting Trials，CONSORT），observational studies（Strengthening the Reporting of Observational Studies in Epidemiology，STROBE）and systematic reviews and meta-analyses（Preferred Reporting Items for Systematic Reviews and Meta-Analyses，PRISMA）.Empirical② evidence suggests that a journal’s adoption of the CONSORT statement as a guide to authors is associated with an increase in the completeness of published randomized trials.

During 2011-2012，a group of clinicians，researchers，and journal editors developed recommendations for the accurate reporting of information in case reports that resulted in the CARE Statement and Checklist.They were presented at the 2013 International Congress on Peer Review and Biomedical Publication，have been endorsed by multiple medical journals，and translated into nine languages.

The CARE guidelines were designed to increase the accuracy，transparency，and usefulness of case reports.They support the efforts of the Equator Network to improve the transparency and accuracy of health research reporting.Online training in writing case reports following the CARE guidelines is available from Scientific Writing in Health and Medicine（SWIHM）.

Each item from the CARE checklist is explained and accompanied by published examples in CARE Guidelines for Case Reports：Explanation and Elaboration Document，which was worked out by a group of experts in 2017.③ The explanations and examples in this document are designed to support the writing of high-quality case reports by authors and their critical appraisal ④ by editors，peer reviewers，and readers.

The 2013 CARE Statement，Checklist，and the Explanation and Elaboration Document，available from the CARE website ［www.care-statement.org/］and the EQUATOR Network ［www.equator-network.org/］，are resources for improving the completeness and transparency of case reports.

The reporting guideline for case reports includes a 13-item checklist（Table 1-3-1）.The primary items of the checklist are title，key words，abstract，introduction，patient information，clinical findings，timeline，diagnostic assessment，therapeutic interventions⑤，follow-up and outcomes⑥，discussion，patient perspective and informed consent.

Authors are required to provide the information of this checklist in their care report articles.
